Case Management Services

This service can be customized to the individual needs of the referral agent and the client and can include a variety of services. Client assessment, communication with relevant stakeholders e.g., employers, physicians, treating health professionals and coordination of referral to relevant services can be provided. It can include return to work monitoring and coordination. The overall goal of this service is to facilitate and promote quality and cost-effective outcomes for both the client and referring agent.

Additional features:

• We can identify barriers to recovery in all areas of a client’s life including physical, psychosocial, spiritual, and environmental concerns.
• We can identify barriers to optimal recovery in all activities the client performs including work, leisure, homemaking and self-care activities.
• We can objectively identify a client’s needs with regard to home support services, attendant care needs and job accommodation needs.
• We can provide strategies to facilitate return to optimal function with the use of adaptive equipment, modified techniques, activity pacing/planning and other strategies as indicated.
